Ancillary Request Form

All organizations looking to request space for an ancillary meeting must submit requests through this form. It is CIS Policy that any ancillary meeting not conflict with the educational sessions of the Annual Meeting. Please refer to the Annual Meeting Agenda for these times.

CIS must approve all ancillary meetings being held in relation to the CIS Annual Meeting. Once a meeting is approved by CIS, the requesting organization will work directly with the Annual Meeting hotel to manage room assignments, and any AV and Food and Beverage needs. Additional fees may apply.
